Abstract

The management of patient health records is a crucial component of today's healthcare systems. However, the traditional methods of storing medical records primarily on paper or in isolated hospital databases are frequently inefficient, unreliable, and difficult to access across a wide range of healthcare providers. Data loss, repeated medical tests, a delayed diagnosis, and compromised patient care are all possible outcomes of these restrictions. To address these challenges, this paper proposes a Blockchain and QR Code–based National Digital Health Record System that ensures secure, reliable, and instant access to patient medical data. Health records are stored in a decentralized and tamper-proof manner using the proposed system, which makes sure data integrity, transparency, and protection against unauthorized modifications. Each patient is assigned a unique QR code that serves as a digital key to their medical history. The efficiency and accuracy of medical services are enhanced because authorized healthcare professionals can quickly retrieve patient records, such as prescriptions, lab results, and treatment history, by scanning the QR code. The system is implemented using a web-based architecture with a user-friendly interface, backend processing, and secure data storage mechanisms. It gives patients and doctors access based on roles, making it possible to share data without disrupting privacy or security. The proposed solution speeds up decision-making, improves healthcare coordination, and reduces manual paperwork. In general, the system provides a platform for modern digital healthcare management that is scalable, effective, and safe.
